2. One-Time Installment Set-up Fee - $10 per policy term for any policies set up on an installment plan other than Annual or Mortgagee billed.

1. MGA - $25 per policy/per term
2. FIGA - Fees assessed based on date policy was bound

3. Installment - $3 per installment including the initial installment
